December 2nd 2021

Dear Parents,

The NHS Immunisation Team are in school on Monday and Tuesday of next week to deliver Flu and Covid-19 vaccinations on Monday 6th & Tuesday 7th December 2021

They are intending to complete delivery of Covid vaccinations on the Monday, though they will be carrying out vaccinations for Flu across both days.

All children in years 7 to 11 are eligible for the Flu vaccination

Children in years 7 to 11 aged 12 and over are eligible for their first dose of the Covid vaccine in school (Cominarty vaccine from Pfizer Biontech)

Please see the NHS Immunisation Team letter to parents inviting them to consent for their child to receive a vaccination. Please complete the consent form for your daughter or child, child only if you have not done so already.

NHS Immunisation Team letter

Some rules have changed meaning that children who have tested positive for Covid-19 during the past 12 weeks will need to be reviewed by our nurses and are unlikely to be able to receive the Covid vaccine. We will pass all information we have to the Immunisation team.

If your daughter or son has received their Flu or Covid-19 vaccine elsewhere, please complete the form declining the vaccine e and this will over-ride any previous consent.

Thank you for all your support and for working with us to ensure a smooth delivery on the day.

Kind regards

Yours Sincerely,

Sonette Schwartz                                                                                    Charles Joseph
